Guidelines
- Create a user profile and choose a password that contains letters, numbers and special characters (eg. “?!”). Write down and save your password.
- Read the practical information on the portal on how to fill in the application.
- Attach the requested documents to the application. You should attach the Transcript of Records plus a copy of your identity documentation (passport or identity card in Latin alphabet).
Your application provides the basis for the Learning Agreement and the planning on our part of your exchange period. The Application Form also serves as the key tool for the planning and corporation between you and your clinical supervisor.
If you are doing an internship or clinical placement you will have to fill out a Training Agreement.
